Level I - (No Response)

  • Unresponsive to all stimuli

    Level II - (Generalized)

  • Inconsistent, nonpurposeful, and nonspecific reactions to stimuli

    Level III - (Localized Response)

  • Inconsistent reaction directly related to type of stimulus presented
  • May follow simple commands inconsistently

    Level IV - (Confused, Agitated)

  • Disoriented and unaware of present events
  • Bizarre and nonpurposeful behavior 
  • Limited attention span
  • Ability to process information is impaired

    Level V - (Confused, Inappropriate, Nonagitated)

  • Nonpurposeful behaviors unless environment is highly structured 
  • Responds to simple commands fairly consistently
  • Poor attention and initiation
  • Often wanders off or attempts to go home

    Level VI - (Confused, Appropriate)

  • Behavior is goal-directed but still needs structure
  • Responses are approriate to the situation
  • Displays difficulty with new learning
  • Follows simple commands consistently

  • Appears oriented to setting
  • Performs activities of daily living automatically and independently
  • Judgement and problem-solving are poor
  • Lacks insight into deficits

    Level VIII - (Purposeful, Appropriate)

  • Completes familiar tasks for a 1 hour time period independently
  • Overestimates or underestimates abilities
  • Recognizes inappropriate social behavior with minimal assistance
  • Uses compensatory memory devices with intermittent assistance

    Level IX -

  • Shifts between tasks for at least 2 hours
  • Displays depression, irritability, frustration
  • Able to self-monitor in social situations with stand-by assistance
  • Uses assistive memory devices upon request

  • Estimates abilities accurately
  • Appropriate in social situations
  • Completes executive tasks independently
